Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED. PLEASE READ IT CAREFULLY.

The vaccination and health screening programs (the “Wellness Programs”) provided by OnSite Wellness Services, Inc. are designed to promote health and wellness and help you assess your risk of disease and to provide information from OnSite Wellness Services, Inc. (or from the sponsors of OnSite Wellness Services, Inc.’s programs) about common health issues, such as high cholesterol, blood pressure, diabetes, osteoporosis, and others, and occasionally products to treat those conditions.

This Notice of Privacy Practices (”Notice”) explains the limited degree to which we may collect, use, and aggregate personal information about you that you provide to OnSite Wellness Services, Inc. through your participation in the Wellness Programs. OnSite Wellness Services, Inc. respects your personal privacy and is committed to protecting it. OnSite Wellness Services, Inc. is particularly sensitive to the privacy of medical and health information.

Your Acceptance of this Notice

By participating in the Wellness Programs, you signify your acceptance of the terms of the OnSite Wellness Services, Inc. Notice of Privacy Practices. If you do not agree to the terms of this Notice, please do not participate in a Wellness Program.

Information Submitted and Collected: Your Personal Health Questionnaire and Biometric Results

In order to participate in an OnSite Wellness Services, Inc. Wellness Program, you may complete a personal health questionnaire consisting of a number of questions about your personal and family medical history related to heart disease, diabetes, osteoporosis, and other common diseases. We will use this information during your clinical screening or vaccination to help assess your personal health. After the screening or vaccination, your responses to the personal health questionnaire and your biometric results from the clinical screening may be used by OnSite Wellness Services, Inc. for the following purposes:

 conducting market research to improve the programs, products, and services offered by OnSite Wellness Services, Inc.;

 to advance the understanding of pathophysiology and therapeutic approaches to treating disease; and

 to provide you with information about common health issues, such as high cholesterol, glucose, blood pressure and products to treat those conditions, which may be delivered to you by OnSite Wellness Services, Inc., or one of their business partners (as described below in the section “Sharing of Information”).

Sharing of Information

OnSite Wellness Services, Inc. is committed to keeping your information private. OnSite Wellness Services, Inc. does not rent, sell or otherwise distribute your private information to third parties with the exception of the circumstances listed below:

• Provision of service: OnSite Wellness Services, Inc. may share your private information with third parties to provide you with a product or service that you have requested from us.

• Disclosure in line with consent: OnSite Wellness Services, Inc. may share your information only upon your consent with designated third parties such as your company’s wellness program vendors and partners. Such disclosure is detailed on the Informed Consent that is presented when service is rendered. Participation is voluntary.

• Disclosure for legal reasons: We may release private information to third parties: (1) to comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order; or (2) in special cases, such as a physical threat to you or others, a threat to homeland security, a threat to our system or network, or cases in which we believe it is reasonably necessary to investigate or prevent harm, fraud, abuse, or illegal conduct. In the event that we are legally compelled to disclose your personal information to a third party, we will make reasonable efforts to notify you unless doing so would violate the law or court order.

• Agents and contractors: Persons or organizations working as an agent or contractor of OnSite Wellness Services, Inc. may have access to your private information for the purpose of providing a requested service and operating our business. Our contractors and agents are required to use your private information for the sole purpose of their work with OnSite Wellness Services, Inc. and not in any other way.

• Aggregated, non-personal information: We may combine information provided to us for the purpose of reporting and analysis. This combined information contains only non-personal information.

• Changes in our corporate structure: If all or part of our company is sold, merged or otherwise transferred to another entity, the personal information you have provided at this site may be transferred as part of that transaction.

Voluntary Participation and Withdrawal

You can choose not to complete the health questionnaire. If you complete the questionnaire and later change your mind and withdraw your consent, we will block the release of any personally identifiable health information about you and your family that can be used to identify or contact you in the future. However, after blocking your name and other personal identifiers, OnSite Wellness Services, Inc., its successors in interest, may keep, use, and sell any such de-identified information that you may have provided for any business-related purpose.

Duration of Your Consent

Unless you contact OnSite Wellness Services, Inc. to withdraw your consent, your health questionnaire information will be stored and used internally by OnSite Wellness Services, Inc. indefinitely.

Security

OnSite Wellness Services, Inc. wants your personal information to remain as secure as reasonably possible. Maintaining a secure system is in the best interests of our customers, program participants and partners. As such, OnSite Wellness Services, Inc. stores personally identifiable data using industry standard physical, technical, and administrative safeguards to secure data against foreseeable risks, such as unauthorized use, access, disclosure, destruction, and modification.
